PDH CHSP Social Services Consultation

Portland District Health invite you to our upcoming consultation session for the planning and co-design of our social support services, both groups and individual supports, with morning tea provided.

Whether you are a prospective participant, volunteer, supporter, or user of the social support program, this is a fantastic opportunity for you to have a say and share your thoughts and ideas on how our program can meet the needs of the participants.

Social Support is the planned activities that are funded under the Commonwealth Home Support Program to support and assist older people to participate in community life and feel socially included through structured, group or individual based activities that develop, maintain or support independent living and social interaction. It can include group- based activities, group excursions, or online activities, or individual services such as visiting services, telephone and well-being monitoring services or accompanied activities (e.g. 1:1 shopping, telephone monitoring services, attendance appointments).
